Dialectical behavior therapy was built for people whose emotions arrive faster and harder than most, and who have found ways to survive them that cost too much. It has the strongest evidence of any treatment for chronic suicidality and for borderline personality disorder, and the skills travel well beyond both.
Adherent versus informed, which is the whole question
A full DBT programme has four components: weekly individual therapy, a weekly skills group that runs like a class, phone coaching between sessions, and a consultation team behind the therapist. That’s what the research studied.
Plenty of clinicians offer DBT-informed individual therapy instead — the skills, without the structure. Legitimate, cheaper, more flexible, and a considerably weaker intervention for active self-harm or chronic suicidal behavior.
Four questions sort it out: do you run or refer to a skills group; do you offer phone coaching and what are the rules; are you on a consultation team; and what is your DBT training. A clinician on a team will name it without hesitating. The fuller version is here.
Where it’s in the Bay Area
Academic medical centres and university-affiliated clinics run the most reliably adherent programmes, and frequently at lower cost than private practice. Several independent DBT-specialty clinics operate across San Francisco, the East Bay and the Peninsula.
County behavioural health services fund DBT in several Bay Area counties, because it reduces hospital admissions and saves money. Waits can be long and cost can be near zero, and this route is chronically under-used by people who assume they won’t qualify.
The DBT-Linehan Board of Certification publishes a directory of certified clinicians and certified programmes — small, and the most reliable list there is.
Cost
A programme is two services billed separately. Individual therapy at Bay Area rates, plus the group, commonly $60 to $150 for the two-hour session. Full private-pay programmes here frequently run $900 to $1,800 a month, and a year of it is a serious number.
The under-used option: a standalone skills group. Many programmes run groups that accept outside members whose individual therapist is elsewhere. You get the whole curriculum at a fraction of programme cost, without changing therapists, and almost nobody is told this exists.
The video option
Remote DBT holds up well in the evidence, attendance is better without a commute, and since any California-licensed clinician can see you, the pool of adherent programmes available to you is the state rather than the county.
